Substance Use Disorders and Mental Health Care Support
At BREEZE HEALTH, we use “reliable clinical improvement” to measure changes, which means that an individual
in treatment has improved enough on a measure of clinical symptoms that we don’t believe the change can be accounted for by normal variation in scores or measurement error.
Our strategy for further development of the product/service is to use machine learning techniques applied to responses and generated data to help move patients along in their treatment plans toward maintaining longevity in long-term sobriety.
Provider Services
- Support may include intensive outpatient programs (IOP), partial hospitalization program (PHP) or rehabilitation facilities.
- Our collaborative care model closes the gap with the patient and your Wellness coaches, therapists, and psychiatrists. this design ensures that patients are provided with the appropriate level of care when they need it the most (regardless of where they physically are).
- Breeze Health is designed to monitor Substance Use Disorders and Mental Health Care issues and to identify when patients are experiencing anxiety, depression and other mental health related issues which could lead to a relapse.
- We use predictive analytics, in order to identify when patient is at risk of abusing and intervene (either human or automated) based on AI recommendation in the platform.
- BREEZE HEALTH is excited to partner with Providers across the United States. We provide assessment tools to make your clinic operate more efficiently thus enabling you to provide the best care for your patients.
Payer Services
- In preparation for a shift to value-based care the BREEZE HEALTH's services are intended to be bundled with other related services. These services are provided by facilities and hospitals as a service subsequent to intensive inpatient, or as part of and after intensive outpatient rehabilitation treatment.
- Services can be prescribed and utilized as an extension of assistance to patients, to encourage adherence to their structured care plan in the mitigation and prevention of relapses and improvement of outcomes.
- These bundled services may be billed by hospitals and rehabilitation clinics and paid for through their patient’s payer channels, such as health insurance providers or government plans like Medicare and Medicaid.
