Intervention, encouragement and passive data collection
- We are able to put personalized digital healthcare tools in the hands of the patient, providing them more protection and encouragement.
- Our analytics tools and data collection tool, bring much needed visibility to the Provider as to what is going on with their patients away from the facility!
- The BREEZE HEALTH program is designed to monitor Substance Use Disorders and Mental Health Care, by identifying when patients are experiencing anxiety, depression and other mental health related issues which could lead to a relapse.
- Our web-based platform and mobile apps are in place for uploading data to your health care team (Provider portal).
- The mobile app passively and actively collects data, sends reminders/notifications, provides encouragement and a variety of other customized alerts as options.
- A Wellness Coach portal which includes a CRM and Care Management System wherein the Wellness Coach conducts periodic scheduled and triggered phone and messaging encounters, care plan updates, assessments, interventions, goals, encouragement, etc.
- The population analytics tool will establish behavioral profiles and predictive scoring. The data is passively collected, providing Providers with much needed intelligence and patient information.
Clinicians Who Care
Intervention is the key ingredient! The sooner a potential relapse is recognized, the higher the likelihood of keeping the patient on track to sobriety.
When we see patients showing declines, slippage, or issues of concern we flag the case for review by a nurse supervisor.
All changes/issues are then passed along to the facility's designated team lead.
Clinicians will now also be able to utilize our technology in an effort to activate and maximize the patient's prevention strategies.
Experience and Professionalism
Understanding and treating Substance Use Disorders and Mental Health Care can clinically be a challenge even in a tightly controlled environment.
Through our secure reinforcement technology, BREEZE HEALTH can support all of the good work done by the facility's Clinicians. When needed, we can issue exception-based alerts and notifications via the phone, text, video and/or web chat.
Navigating a patient through these diseases can be extremely difficult, especially when a patient needs urgent help when away from the facility and prone to a relapse. When we identify that a patient is at risk of abusing, we are able to intervene (either human or automated) based on an AI recommendation in the platform.
